Our Intake Process

This is what to expect with our intake process:

1. Use the “Request an Appointment” button to access our online calendar where you can book a free 15-minute phone consultation with our wonderful Intern Therapist, Carley Ritz.

   If none of the times work work with your schedule or if you have any questions, please contact admin at crmhinfo@gmail.com or 780-781-8980.

   PLEASE NOTE: These appointments are completed via telephone.

2. Complete the free 15-minute Initial Consultation. Once completed, Carley will refer you to a clinician of best fit at CRMH or provide external referrals.

   PLEASE NOTE: If you have a conflict of interest with Carley Ritz, please email crmhinfo@gmail.com and request an initial consultation form to be reviewed by Dr. Kendell Banack, or contact a therapist directly using the appropriate emails on the bio pages.
